What NAD+ is (and why you have never heard the whole story)
If you know NAD+ at all, it is probably from a reality-TV clinic scene or a friend describing a four-hour IV drip at a wellness spa. That is a strange introduction to a molecule that has been in biochemistry textbooks for more than a century. Nicotinamide adenine dinucleotide was identified in 1906 by two British scientists studying how yeast ferments sugar, and for most of the years since it was filed under "metabolic cofactor," essential and unglamorous.
Here is the plain version. NAD+ is a shuttle. It picks up electrons from the food you break down and hands them to your mitochondria, which use them to make ATP, the energy currency every cell spends. Every heartbeat, every thought, every mile walked runs on that hand-off. But the picture for women specifically is more interesting than "it drops." In 2022 a Chinese group measured NAD+ in whole blood from more than 1,500 people and split the results by sex. Men showed a steady decline with age, steepest after 60. Women did not show the same pronounced downward slope; their blood levels stayed comparatively stable across age brackets. It is one dataset, blood rather than tissue, and it does not tell us what is happening inside your muscle or brain. Still, it is the largest direct human measurement we have, and it should make you skeptical of anyone who tells a 42-year-old woman her NAD+ is "half gone."
Each dot is the average blood NAD+ for one age bracket. The left panel (men) slopes downward; the right panel (women) is much flatter. This does not make NAD+ irrelevant for women. It means the "half gone by 40" claim rests on tissue and animal data, and what blood cannot show is the demand inside cells from sleep debt, training, and hormonal transition.

So why do so many women in their late thirties to fifties describe exactly what NAD+ is marketed for: the 3 p.m. wall, the word that will not come, waking tired after a full night in bed? Because NAD+ is consumed by demand, not just the calendar. Enzymes that use it get busier under cellular stress, and CD38, an enzyme that breaks it down, becomes more active with age and inflammation. Short sleep, hard training, a demanding job, the perimenopause years when sleep gets less predictable for many women: all of it raises the draw on the same pool. In general terms, a woman can have a normal blood level and still have tissues running closer to the line than at 28. That is the honest case for NAD+, and it is narrower than the marketing version.

Tier two: human trials of precursors
The best human data is on oral precursors, the raw materials your cells convert into NAD+. Nicotinamide riboside (NR) taken for six weeks by healthy middle-aged and older adults raised NAD+ in circulating blood cells by about 60 percent and was well tolerated. In a small trial that matters for this audience, NMN improved muscle insulin sensitivity in postmenopausal women with prediabetes, though most other measures did not change. Precursors reliably raise the blood number. Whether that translates into feeling different is where the evidence softens, and the honest answer is "sometimes, modestly."

Tier three: direct NAD+ by IV or injection
Here the literature is thinnest. The best-known study is a 2019 pilot in which volunteers received a six-hour IV infusion of NAD+ while researchers tracked it in plasma and urine. Plasma NAD+ barely moved for two hours, then rose, and part of the dose left in urine as breakdown products. It showed the body takes up and processes infused NAD+, and that a fast bolus is partly wasted. It did not measure how anyone felt. Subcutaneous injection is a well-established route for peptides, and its slower absorption is the argument for it over a drip. But there are no large randomized trials of subcutaneous NAD+ on energy or focus in women. What we have is mechanism, precursor data, and clinical experience.
